Typical titration example: Wegovy®
Wegovy's manufacturer dosing guidance describes a gradual escalation, typically increasing every 4 weeks, with maintenance commonly at 1.7 mg or 2.4 mg weekly depending on response and tolerability.
Medication-Assisted Weight Loss
GLP-1 medications can be powerful tools for weight management, but they work best when they are part of a complete, physician-guided plan.
At Rochester Medical Weight Loss, your care is in person and physician-guided, with medical baseline testing and ongoing follow-ups so your plan can be adjusted safely over time.
What makes treatment safer: nutrition strategy, clinical monitoring, side effect management, body composition tracking, and long-term maintenance planning.
How your care begins matters
Many online weight loss platforms are built for speed. GLP-1 treatment should be built for safety, context, and long-term results.
Some online platforms rely on a questionnaire, basic history, portal messaging, and a prescription. In many cases, there may be no physical exam, vitals may not be directly measured, labs may not be required, and body composition may not be assessed.
At Rochester Medical Weight Loss, GLP-1 therapy starts with a comprehensive physician-led evaluation because obesity is complex and treatment should not be guesswork.
Before you start
Before prescribing or continuing GLP-1 therapy, we evaluate the full picture so treatment can be safer, more personalized, and easier to adjust over time.
A medical baseline helps us personalize dosing, reduce avoidable side effects, and build a plan that supports both results and safety.
Progress is not judged by the scale alone. Body composition tracking helps show whether changes are coming from fat, muscle, or water.
Starting & titration
Most GLP-1 medications are started at a low dose and gradually increased. This "start low, go slow" approach is designed to reduce gastrointestinal side effects.
Wegovy's manufacturer dosing guidance describes a gradual escalation, typically increasing every 4 weeks, with maintenance commonly at 1.7 mg or 2.4 mg weekly depending on response and tolerability.
Zepbound commonly starts at 2.5 mg weekly for 4 weeks, then increases to 5 mg weekly, and may be increased in 2.5 mg steps after at least 4 weeks at each dose, up to a maximum of 15 mg weekly. Maintenance dose depends on indication and tolerability.
Important: If side effects flare during escalation, clinicians may hold the dose longer before increasing.
Side effects
The most common side effects are gastrointestinal and often show up early in treatment or around dose increases.
Most common side effects may include nausea, vomiting, diarrhea, constipation, acid reflux, and abdominal pain.
Your plan can be adjusted if symptoms flare. This is one reason follow-up and communication matter.
Nutrition support
A common mistake on GLP-1s is under-eating and unintentionally losing lean mass. Another common issue is letting the medication replace the plan.
Without body composition analysis, you cannot tell the difference between fat loss and muscle loss. Losing muscle can make long-term maintenance harder.
If you are increasing activity, you may lose fat while preserving or building lean mass, and the scale may not reflect that clearly. Body composition tracking helps show what is actually changing.
